I use the Permalink Redirect plug-in by Scott Yang and it works beautifully well with few known conflicts with certain plug-ins. I’m using the permalink structure /%postname%/%post_id%/ cos I hear it helps reduce conflicts on wordpress blogs and also helps speed things up.

Please note that your permalinks cannot change when you modify you post titles UNLESS you modify them yourself. Try it and see for yourself

I use the permalinks:

/%post_id%/%postname%/

And I use the Clean Slugs plugin from Yoast. It removes all unnecessary small words from your domain.
